  • (2)Get a sheet of paper, (Or, just Click here to print this Order Form)
  • (3) List the items by description you want. Need more room? Print two forms, add them up..
           (Item#, Description, Price, then add $8.00 for packaging and postage),
  • (4) Total the dollar amounts for each page (only one packaging cost on the entire order, within reason),
  • (5)Send Order Form (via USPS snail mail), with your check or Postal Money Order, to:

    Please make sure to write your items legibly, and add up the prices, postage, and final total accurately. If you write carelessly, and we can't read your writing, we'll either send you what we believe you meant, or ask you via your eMail address. If we are out of what you order, we'll contact you via your eMail address to ask if you want a substitute item.

    Personal Checks must clear our bank before we ship. Postal Money Orders and Certified Checks rate shipping the same day we cash them. No returns, all sales final.

    IF YOUR SHIP-TO ADDRESS IS NOT IN THE U.S.A., postal rates, customs requirements and restrictions are rather complicated and time consuming. Please, inquire by before you send any orders for merchandise to be shipped outside U.S.A.
